FBS Employee Agreement

I AGREE to the following terms and conditions while receiving child care benefits through United Way of Weld County’s Family Business Solutions.

1. I agree to provide in writing, using the “Change of Service” form, any changes listed below:
      –  Change in name, address, and/or phone number
      –  Change in my child care provider
      –  My child is no longer receiving child care services
2. I understand my child care benefit:
      –  is directly related to my employment
      –  will be paid directly to my child care provider
      –  will be used for the company approved amount per child only and not for any extra fees/charges or back pay
3. I agree to pay all child care charges incurred above the approved child care benefit amount.
4. I understand Family Business Solutions will notify me regarding any changes in the amount of the child care financial support, or other program changes.
5. I understand I am responsible for selecting my child’s provider. I may access the names of licensed providers online at www.ChildCare4Weld.org or Colorado Shines or through Child Care Resource and Referral Services (CCR&R) at 1-877-338-2273 or. I understand CCR&R does not evaluate, screen, or recommend child care providers and the state licensing process in no way implies a quality relationship between the child and caregiver.
6. I accept it is my responsibility to select and monitor my child’s care, and to work out problems with my child care provider.
7. I recognize participating businesses contribute to United Way of Weld County’s Family Business Solutions program with the intention of attracting and maintaining a stable, dependable employee base. The continuation of this depends on contributions and the program’s impact on improving productivity and retention.
8. I authorize United Way of Weld County employees to contact my past and present child care provider(s) and the Department of Social Services when applicable, to confirm the child care information I provided. United Way of Weld County will maintain a high level of confidentiality concerning information is not deemed to be public.
