Thank you for taking part in this year’s professional development day. We are thrilled to have you on the agenda. Please take a few moments to provide us with information regarding yourself and the presentation by answering the questions below. This information will allow the participants to attend the sessions most relevant to them. We also use your responses to create the conference booklet, which includes the agenda and session descriptions along with your amazing photo and biographical information.
Also, please let us know if you will be needing any other items for your presentation, such as sticky notes on the table, copies, highlighters for participants, etc. There will be technology available in every session but if you would like a Power Point printed for yourself and/or participants, for example, please let us know ahead of time using this document.
Finally, please provide us with an approximate cost for your participation, estimating multiple sessions individually if you have offered to present for more than one session. Also, for a line item not listed, please add it with a description and cost. Lunch will be provided to all presenters.
