The culmination of the Project is a notebook . This notebook contains information that is frequently required by an individual's Physicians & Specialists Case Management/ Therapists/ Insurance Companies. The notebook is designed to condense thick medical files into an easy reference system. It is this system that brings a physician up to date in a remarkably short period of time. The notebook provides instant access to medical information as described by other physicians evaluations, rather than relying on a person's memory during a crisis. it supports an individual's ability to communicate effectively and helps the physician to focus on current concerns. The mission of Patient Profile Project is to empower individuals and their families to manage their health care needs and be more effective advocates in the health care system.
