Program/Service Notes:
Provides a central registry for people who want to donate their organs / tissues at the time of death. People who are interested in becoming donors can request donor information forms on which they specify which organs and / or tissues they will donate and any limitations or special wishes. The document must be signed by two witnesses. Donors receive a donor card which is carried by the individual at all times and which indicates the person is registered with the Living Bank. The organ procurement organization makes the decision regarding what can and cannot be used for transplant. For whole body donations, the individual should contact the Living Bank. The Living Bank includes a newsletter which has news stories on programs and events, research notes and letters.
