205 E. 6th St.- Loveland, CO-- 1601 Brodie Ave. - Estes Park Provides reimbursement for transporting Medicaid eligible recipients to and from medical services that are covered by the Medicaid program. Reimbursement forms must be submitted for processing in order to be reimbursed. Special accommodation : handicapped accessible and proof of legal residency required.
