Program/Service Notes:
Provides individual residential services and supports in community based non-congregate living. Supports may be 24 hour on site staff, scheduled support staff, room mates and host homes. Services include all skills for daily living, money management, health and safety maintenance, personal care, access to community services, interpersonal relationships, recreation and transportation.
