Program/Service Notes:
This program supports families who have children between birth and 3 years of age with special needs. The program provides support to families by identifying funding sources for services, develop an IFSP (Individualized Family Service Plan); ensure families have a Service Coordinator; ensure families have information in their native language. A service coordinator is a person who can assist families, navigate systems and identify resources that will meet the unique needs of their child.
